Rehabilitation following cardiovascular (CV) diseases and interventions is a priority for all health care systems. The project has developed an integrated mobile phone application for close and efficient monitoring of patients' condition. The application will enable the follow-up of discharged patients, optimisation of their lifestyle and can increase the effectiveness of CV prevention. It provides medication monitoring, medical control consultation management, indication of physiotherapist instructions and patient monitoring together with automatic and manual alarms.
It implements the steps of continuous patient tracking by continuous input of general data (temperature, blood pressure), recording of other specific data (ECG, wound photo) and indication of complaints. A further key data point is the measurement of movement activity by mobile phone, which can be used to infer the progress of the disease.
The application is based on the elements and functioning of hospital rehabilitation care as a summary system, with modules that can be developed independently and extended with other phone applications and wearable sensors.
